Are You A Victim Identity Theft?

01

Report Identity Theft

The only way to inform the IRS that you are a victim of identity theft is to report it with Form 14039.   You can fax the completed form to the IRS – download with the following link:

https://www.irs.gov/pub/irs-pdf/f14039.pdf

 Or

Use this online submission form: https://www.irs.gov/dmaf/form/f14039

The IRS automatically issues a PIN that is needed in the future to submit any e-file returns.  This prevents unauthorized use of your social security number in e-filing your tax return.

03

Tax Return Filing Verification

You can verify if someone filed a tax return with your information at 1-866-682-7451 extension 568.  You can then report identity theft to the IRS.

02

Identity Protection PIN (IPPIN)

If you are not a victim of identity theft but would like to obtain an Identity Protection PIN (IPPIN), here is the link to obtain an IPPIN online:

You will need to set up an ID.ME account to confirm your identity if you have not already established one already.
